Online Liability Waiver Instructions

Pelham Fish and Game Club has transitioned to an electronic online system for completing and submitting liability waivers and collecting guest fees. A waiver must be completed by any guest entering the property. A $10 fee must be paid for any guests aged 18 and older.

The electronic waiver is essentially identical in content to the former paper waiver with the addition of an email field and payment capability.  This allows the system to send a copy of the completed form and a receipt for any payments to the email address provided.

The form must be filled out and signed by the GUEST (or parent/guardian for minors). It can be filled out online using any web browser, tablet, or smartphone. This can be done prior to arriving at the club or upon entering the premises. The other option is to use our iPad based kiosk setup in the hallway outside the bathrooms in the main clubhouse. You can use you badge to enter that area at the rear doors. To activate the kiosk, touch the iPad screen and then swipe up from the bottom of the screen.  This should then display the form for you to complete.  If there are issues with the kiosk, instead use your smartphone to complete the waiver. There is a QR code that can be scanned to bring up the waiver form or find it on the club website.

Form Completion Instructions:

  1. Read the text of the liability waiver.

  2. Click whether the form is being completed for an adult or a minor. If it is for a minor, the parent or guardian must complete the form.

  3. Enter your name, address, phone number, and email address. If the form is for a minor enter their name in the "Name of Minor" field.

  4. Sign the form.  On a tablet or smartphone you can use your finger to write your signature. On a computer you may need to use the mouse or touch pad to fill the signature box.

  5. The Date field should already be filled with the current day but you can change it if necessary.

  6. Guests must be accompanied by a Regular or Senior member. Enter the name and badge number of the member. Remember, no more than two guests per Regular or Senior member.

  7. A $10 fee per guest, aged 18 or older, must be paid with the waiver. You can choose to pay for multiple guests on a single waiver form, rather than doing multiple transactions.

    • Choose whether you will be paying for one or more guest fess on the waiver. If you will be paying the fees on a different waiver choose that option.

    • Select the number of fees you will be paying. It will default to 1.

    • Enter your credit card information. On some web browsers you may also have the option of using Apple Pay or Google Pay.

  8. Once the form is complete click the green "Submit Waiver" button at the bottom. If there are any errors or missing required fields they will be highlighted and must be fixed before it will be accepted. You will be redirected to a successful submission screen and the process is complete. A copy of the completed waiver and a receipt for any payments made will be immediately emailed to the email address provided. If you completed the waiver at the PF&G Kiosk, a paper receipt will be printed. Please take this receipt and keep in on you while at the club.
